To request a copy of your criminal history, you must file a request in person at the West Valley City Police Department located at 3577 S Market Street (Public Safety Building north of City Hall; enter on the southwest side). Although you may print a Criminal History Request form (PDF) from our website, you must sign the application and a Waiver of Liability in the presence of a West Valley City Police Department employee - No Exceptions.

The fee is $15.

Identification (ID)

We require valid government photo identification such as:

  • Alien Registration Card
  • Driver's License
  • Military ID Card
  • Passport
  • State Identification Card

This does not include a Utah State Driving Privilege card.
